High temperature with abdominal pain

A high temperature (fever, 38°C or above) together with abdominal pain is a combination that is worth checking. It can be from a common stomach bug — but it can also be a sign of something that needs treatment, such as a urinary or kidney infection or appendicitis.

When to get medical help

Use NHS 111 online, call 111, or contact your GP if:

  • Fever plus abdominal pain for more than 24 hours
  • Pain that starts around the belly button and moves to the lower right — could be appendicitis
  • Pain in the side or lower back with a fever — could be a kidney infection
  • You are pregnant
  • You have a long-term condition affecting your tummy

Call 999 or go to A&E if you have severe abdominal pain that is getting worse, a rigid tummy, vomiting blood, pain that stops you moving, or you feel faint.
